Participation Waiver, Release of Liability, Assumption of Risk & Indemnification Agreement

PLEASE READ CAREFULLY. THIS IS A LEGALLY BINDING DOCUMENT.

Instructor Disclosure: Babasemilore "Semi" Opadele (age 17) and Babadarasimi "Dara" Opadele (age 13) are minor-aged, teenage basketball players providing informal youth basketball instruction. They are NOT certified coaches, licensed trainers, medical professionals, or childcare providers.

Assumption of Risk: Basketball training involves INHERENT RISKS of physical injury including sprains, strains, fractures, concussions, and other serious bodily injury. I VOLUNTARILY ASSUME ALL RISKS, both known and unknown, even if arising from negligence of the Released Parties.

Release of Liability: I release, waive, and discharge Babasemilore Opadele, Babadarasimi Opadele, the Opadele family, Baba B-Ball, the Baba B-Ball coaching staff and affiliates, Christian Herbie Clarke (website designer), Shopify, Inc. (payment processor), any venue, church (including the Church of Jesus Christ of Latter-day Saints), facility owner or operator, organization, and all other associated parties (collectively, the "Released Parties") from ANY AND ALL liability, claims, or damages.

Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from any claims, suits, or expenses.

Venue Disclaimer: Sessions are held at third-party facilities in Hyde Park, Chicago NOT owned or controlled by the Released Parties. Neither the Released Parties nor any facility owner, church (including the Church of Jesus Christ of Latter-day Saints), or organization accepts liability for any injury or loss at these venues.

Parental Supervision: A parent or legal guardian MUST remain physically present during ALL sessions.

Medical: I confirm my child is physically fit to participate. In emergencies, I authorize the instructors to call 911.

No Insurance: Baba B-Ball does NOT carry liability or accident insurance.

Governing Law: This agreement is governed by the laws of the State of Illinois.
